Fee for a valuation of accrued sick days

We accept VISA, MasterCard, money orders, certified cheques and personal cheques. To pay by VISA/MasterCard, complete the following form.

We would be happy to have a telephone conversation to discuss the results of the report and answer any questions that you may have, at no extra charge.

 

Process and forms for a requesting a valuation of accrued sick days

In order to have your accrued sick days valued you will need to follow these four steps:

1. Provide us with information describing the accrued sick day program with your employer.

2. Provide us with the number of accrued sick days at the date of marriage and at the date of separation (you will generally need to go to your employer for this).

3. Please send us this information by mail, email (a scanned copy) or by fax.
